That's WordPress making comments second-class citizens again. The filter part is relatively easy to fix, but there's supporting infrastructure to build too, like a connection to the media library as in the full post editor. I want to fix it but can't right now, is the bottom line. For now you're going to have to plan your posting to post images in root topics only. Sorry about that. They seem to be filtered out.
